a titan from them?

same answer:
Because the weapon was fired at a wrong image of the Titan. The missile
does not hit it, flies past him and explodes in a distance. Got the idea?
The gunner tries to place the "template" (point of impact) where he
guesses the Titan is. He may guess wrong.

second reason: because every unit with a save (like Terminators) get it
and the holofield is nothing else (in rule terms, only 2+ rather than 4+).

Holofields got simplified in the proces of creating Epic.
