This is the way I play cover in general :
When you have a LOS but the targeted unit can't be seen completely then
this unit is in cover.
Example : a unit in the ruins
If it can be seen completely it isn't in cover.
Example : a unit in front of the ruins
If it can't be seen at all (no LOS), then you can't shoot.
Example : a unit behind the ruins
